Henry Mayo Newhall Hospital in partnership with American Medical Response (AMR), is improving the transition of care for the Santa Clarita Valley community. Under the new Community Care program, Henry Mayo’s transitional care nurses work with AMR community care coordinators to provide a high level of care to patients who are transitioning from hospitalization to home.

The goal is to improve patient health outcomes, improve patient satisfaction, and ultimately reduce hospital readmission rates. The program offers a more responsive option to potentially time-critical evaluations, deployment of home-based monitoring and rapid resolution of non-medical challenges that can often lead patients back to the hospital.
“We are excited about our collaboration with AMR and the development of the community care program to better support our patients as they transition from Henry Mayo to their homes,” said Elizabeth Chow, Director of Case Management at Henry Mayo. “Our goal is to provide the necessary support and access to resources to improve the transition thus reducing the occurrence of unnecessary readmissions.”
Under the program, transitional care nurses meet with patients during their hospitalization who are at a higher risk of not receiving the support they need at home and thus at a higher risk of incurring preventable readmissions. The nurses follow up with phone calls to these patients at home to review their discharge instructions; make sure they have appointments to follow up with their physicians, and that they have the resources they need to have a successful recovery.
Community care coordinators, who are EMTs with AMR, provide additional support and resources to patients needing more than a phone call.  This support ranges from transportation needs to home safety checks.
